The prostate gland is a walnut-sized gland located below the bladder, surrounding the urethra. It produces components of seminal fluid and plays an important role in ejaculatory function. Three main conditions affect the prostate: benign prostatic hyperplasia (BPH), prostatitis (inflammation), and prostate cancer. Together they account for one of the most common reasons men over 40 attend a urology clinic.

Regular prostate health assessment is recommended for all men from age 50 (or from age 40 in men with a family history of prostate cancer or African ancestry, who have higher baseline risk). A PSA (Prostate Specific Antigen) blood test is the primary screening tool for both prostate cancer and BPH. At Urogyn Busan, PSA results are available on the same day, allowing the urologist to discuss findings and next steps in a single visit.

BPH — Benign Prostatic Hyperplasia

BPH is the non-cancerous enlargement of the prostate gland that occurs as part of normal ageing. It affects over 50% of men by age 60 and 90% by age 85. As the prostate enlarges, it compresses the urethra, causing lower urinary tract symptoms (LUTS): urinary frequency, urgency, weak or interrupted stream, incomplete emptying, and nocturia (waking at night to urinate). The IPSS (International Prostate Symptom Score) questionnaire quantifies symptom severity and guides treatment decisions. First-line medical treatment uses alpha-blockers (tamsulosin, alfuzosin) to relax smooth muscle and 5-alpha reductase inhibitors (finasteride, dutasteride) to shrink the prostate over time. Surgical referral is arranged for significant obstructive uropathy.

Prostatitis

Prostatitis — inflammation of the prostate — is categorised into bacterial (acute or chronic) and non-bacterial (chronic pelvic pain syndrome, CPPS) forms. Acute bacterial prostatitis presents with fever, rigors, severe pelvic and perineal pain, and dysuria — it requires urgent antibiotic treatment. Chronic bacterial prostatitis is characterised by recurrent UTIs with the same organism. CPPS (the most common category) presents with chronic pelvic and perineal pain without a clear infective cause and requires a multimodal treatment approach including alpha-blockers, anti-inflammatory therapy, and pelvic floor physiotherapy.

FAQ

Frequently Asked Questions

Common questions from international patients seeking treatment in Busan, Korea.

What is a PSA test and should I get one?
PSA (Prostate Specific Antigen) is a protein produced by prostate cells. Elevated PSA can indicate prostate cancer, BPH, prostatitis, or recent ejaculation. It is used as a screening and monitoring tool, not a definitive cancer test. The UK NHS, US ACS, and EAU recommend informed PSA screening discussions from age 50, and from age 40–45 in higher-risk groups (family history, African ancestry). At Urogyn Busan, same-day PSA results are available with immediate urologist interpretation.
What symptoms suggest I need a prostate check?
See a urologist promptly if you experience: weak or slow urine stream, difficulty starting urination, stopping and starting during urination, feeling of incomplete bladder emptying, urinary frequency (especially at night), sudden strong urge to urinate, pain or burning during urination, blood in urine or semen, or pain in the lower back, pelvis, or upper thighs. Many of these symptoms are caused by benign conditions but all warrant urological assessment.
Is BPH dangerous?
BPH itself is not dangerous, but untreated severe BPH can lead to urinary retention (inability to urinate), recurrent UTIs, bladder stones, bladder damage, and in severe cases, kidney damage from chronic urinary obstruction. Early treatment prevents progression. Most men with BPH are well managed on medication without needing surgery.
What is a prostate ultrasound?
Transabdominal ultrasound scans the prostate through the abdominal wall (requiring a full bladder) to measure prostate volume, assess morphology, and check for suspicious nodules. Post-void residual measurement follows immediately. At Urogyn Busan, ultrasound is performed by the urologist, allowing immediate interpretation and discussion.
Is prostate cancer checked at Urogyn Busan?
PSA screening as a first-line investigation is available. If PSA is elevated and clinical examination is suspicious, multiparametric MRI (mpMRI) and prostate biopsy can be arranged through our specialist referral network. Early-stage prostate cancer found through PSA screening is highly treatable — regular monitoring is important for men in at-risk age groups.
